  • Creekstone Farms: “I [REDACTED] noticed a offensive odor in the hallways and in the stairway ​that leads to the slaughter floor and the basement. I went to the basement and observed the ​following deficiencies: there was a red inedible barrel which was 1/3 full of decaying matter with ​maggots, 3 large white bins that were also full of decaying matter which had maggots and ​black bugs, maggots were also underneath the bins and all around the area of the bins on the ​floor. The warehouse in the basement had 3 boxes that were opened and the product in the boxes ​was exposed and laying on the floor. There were multiple pieces of tendons scattered on the floor ​from one of the boxes that was opened. There were no QA Tags or tape in the area or on the ​products. In the hallway between SL-1 and SL-2 across from the maintenance work area, there ​were bags of ears that were that were from August 6th, 2021 harvest production date. This issue ​has been brought up before on multiple occasions and meetings about the odor and blood that is ​coming from them and causing an insanitary condition,” according to a noncompliance report dated ​August 9th, 2021, for Creekstone Farms Premium Beef Establishment M27 in Arkansas City. Inside the covering over the ​end to the auger, maggots were observed.
